How We Operate

In our initial stages, we are organizing and providing general information about what Connected Communities are — a generalizable continuum-of-care model that is designed to help local champions figure out how to embed supportive services in neighborhoods where they live. We hope you will read through our information with an eye to figuring out how YOU and your colleagues can collaborate to create a Connected Community in your area!

On January 27, 2027, we’ll be hosting a virtual symposium highlighting success stories of Connected Communities in-the-making all across the country. We hope you will write to us and suggest promising housing-with-services initiatives you may know about, too, so that we can include them on this website.
